The Air Force thought the pressure vest prevented pilots from breathing properly. These two pilots were not wearing pressure vests and had oxygen deprivation. . Pilots need to breathe oxygen.

Another too expensive defense program that does work not well - Navy JSF that cannot land on aircraft carriers, and an air superiority fighters that potentially kills the pilot by oxygen deprivation.
